The coords of the Lair of Death portal are: 42.0N, 58.1E. The nearest lifestone is on the road SW of Arwic at 33.9N, 58.2E.

There is a level restriction of L15 and up which is a decent restriction. The dungeon is tough, but if we're camped there a lower level could survive enough to have a look around. Olthoi are tough to hit though and it may not be much fun for anyone under L20. Also, the Creature AI is smart enough to come after lower-level chars so be warned. I have written on the subject before...

The portal is in a valley just NE --down the hill really-- from the Mite Maze portal (41.1N, 57.3E) which is itself just E of the Mountain Shortcut. To get to the LoD, first attune SW of Arwic (33.9N, 58.2E) and run to the Mountain Shortcut portal (35.0N, 56.0E). Run E up the hill past the Mite Maze portal and down the hill. Near the Mite Maze are usually a couple Outcast Monougas and perhaps a couple Mites. Between the Mite Maze and the LoD entrance you'll usually find a Limestone Golem and a couple mattys or perhaps a couple Bronze 'Dillos. If you continue SE through the valley past the LoD entrance, you'll find some more Matty spawns. The area is generally trafficed and you shouldn't have trouble recovering a body if you happen to die in the area. If you need to get back to Arwic, there is a portal back to Arwic (33.9N, 58.2E) that is just down the hill from where the original Mtn Shortcut portal dropped you.

The room just inside the LoD is relatively safe and has a portal back to the surface in an easily accessible passage. However Olthoi Nymphs frequently are drawn into the room and the occasional Olthoi Worker will be found battling there. The dungeon is pretty well trafficed so expect to find someone there.
